Home care and support services to Bridgetown and Greenbushes

Bridgetown Community Resource Centre (CRC) in conjunction with HomeMade, a commonwealth approved home care package provider, can offer a wide variety of home care support and services so you can continue to enjoy living at home. Whatever your needs, we will work with you to put a plan into action so that you can live your best life at home in your community.

Wherever you live in Bridgetown, Greenbushes or nearby - whatever level of care you require - we will work with you.

A Home Care Package can be tailored to your needs and support you to continue living at home.

Home Care Packages are personalised packages of care designed to help you live a healthy, safe and independent life at home for as long as you choose.

​​​We can help around your home with tasks that are becoming difficult, help you get ready for your day or support you to get back to doing the things you love or need to do.

  • We can help you to apply for an Australian Government subsidised Home Care Package

  • We can coordinate the supply of services you need once you have a Home Care Package subsidised by the Australian Government

  • We cancoordinate the supply of services you need if you are a private client and need support at home

Home Care Packages subsidised by the Australian Government are avaliabe in 4 levels based on your needs

If you are over 65, or an Aboriginal or Torres Strait Islander over 50, you could be eligible for a Home Care Package funded by the Australian Government.

Under each level, we've listed some popular services we offer as examples only. Depending on tthe package you are allocated, you can pick and choose the sevices that suit you and your budget.

Level 1

Worth approximately $10,932* a year for basic needs like:

essential cleaning and social outings

Level 2

​​

Worth approximately $19,224* a year for services like:

Meal preperation, cleaning, gardening and home maintenance

Home Care Package Options

Level 3

​​

Worth approximately $41,847* a year for higher care and support needs such as:

Meal preperation, domestic assistance, personal care, cleaning, gardening and

 

home maintenance, social support, allied health services

Level 4

​​

Worth approximately $63,440* a year for high level care and support such as:

Meal preperation, nursing care and allied health services, personal care

 

domestic assistance, medication support, social support, gardening and

 

home maintenance

Your Home Care Package will provide you with choice, control and flexibility in the way your care and support is delivered

How much will a Home Care Package cost me?

Being eligible for an Australian government Subsidised Home Care Package (HCP) means that the government will subsidise the cost of your care. If you are on a full pension, you will not be asked to make any personal contribution for our home support services if you were assesed as eligible for a HCP prior to 12 September 2024. If you are assesed after this date you may be required to make a contribution to services other than clinical care 

Clinical Governance and Package Management Fee:

These fees help cover nessesities including general management, ongoing assesment and care planning, compliance and quality assurance, management of incidents, scheduling of services, financial oversight and funding, sourcing and ordering equipment and products, additional visits and correspondence as required.

Basic Daily Fee: The basic daily fee is an amount that an organisation may choose to ask clients to pay. HomeMade does not charge a basic daily fee

Income Tested Fee (ITF): The income tested fee is a contribution that some people pay, as determined through an income assesment. This fee is based on individual income, including a part-pension. However, full pensioners and people with an income up to $33,027.80 do not pay an income tested fee.

Co-contributions for pensioners - From 1 November 2025 with the introduction of Support at Home, 5% of cost of independence services and 17.5% for domestic services and gardening.

Co-contributions for Self Funded Retirees - From 1 November 2025 with the introduction of Support at Home, 50% of cost for independence services and 80% for domestic services and gardening.

HomeMade fees are less than the average for service providers
